How to use the Form Template Builder - Web version

Accessing the Form Template Builder

You can access the Form Template Builder from the Forms Templates 2.0 page by doing any of these two actions:

  • Edit an existing template, or

  • Create a new template

Editing an existing Template

To edit an existing template, click the three vertical dots (options menu) on the right side of a published template in the list of form templates:

After clicking on the three vertical dots (options menu), a drop-down button will appear. There, you can create a new draft:

The "Draft" mode is a new concept that is currently being used to illustrate and guide you through the journey on how you edit your template. While a template is in draft mode, any of the changes that you are doing in it are not visible to your project colleagues. In order to make those changes available to your project colleagues, you need to publish your template.

After having opened an existing form template, you will be able to create a new draft:

Note: If you already have an existing draft, you can always edit it. To do so, find it in the form templates list, click on the options menu and choose to open it:

Creating a New Form Template

Click on the button "Add New Template":

When the pop-up appears, you can:

  • Enter a template name

  • Select a form type (e.g. Safety, Health, Environment)

  • Enable/disable signatures. This lets users add signatures when finalizing the form

  • Choose a language so colleagues can find the template in that language later

Click on the "Create Template" button to begin building the form template:

Once the builder opens:

  • The form is divided into Sections. A form template can have as many sections as you wish.

  • Sections are divided into Questions. Each section can contain multiple questions.

a) Click on a section to expand or collapse it:

Note: The last section you expanded or collapsed will be highlighted in green. The section that is highlighted in green is also the section that you are currently focussed on.

b) To collapse all sections at once, click on the "Collapse sections" button:

c) Once the section is expanded, you can view the questions within the section:

Note: The numbers of the questions in the section are being calculated automatically. Naturally, the question numbers respond to the sections they are in.

Template Details

Click the page icon next to the template title to edit the form template’s name, type, or language:

Working with Sections & Questions

a) To insert a new section, click "Add section"

After adding a new section, you will find three buttons in the top-right corner of the section:

  1. Clicking the "Add/edit visibility rule" button which allows you to determine when this section will be shown.

  2. "Collapse questions" button

  3. Clicking the "Options" button shows the drop-down menu below:

    • Clicking the "Show/Hide description" button to show or hide the description of this specific section.

    • Clicking the "Duplicate section" button will duplicate the section.

    • "Delete section" button


​b) To insert a new question, click "Add question"

After clicking the "question" button, a drop-down menu with the following question types will appear:

  • Date

  • Free text

  • Free number

  • Multiple choice

  • Single choice text

  • Single choice number

After adding a new question, you will find two buttons in the top-right corner of the question:

  1. Clicking the "Add/edit visibility rule" button allows you to determine when the question is shown.

  2. Options (drop-down menu)

    • Hide/Show description. Show or hide the description for this question.

    • Allow comments. This lets the person filling the form add comments to this question.

    • Allow attachments. This lets the person filling the form to add attachments to this question.

    • Duplicate question. Creates a copy of the question.

    • Delete question. Removes the question.

Note: You can reorder questions and sections using the arrows next to the question number. The reordering arrows will appear when the section or question is collapsed:

Answer Types and Predefined Answers

Predefined answers can be set for all the types of answers in the drop-down menu: "Date", "Free Text", "Free Number", "Multiple Choice", "Single Choice Text", and "Single Choice Number" answers:

  1. By clicking the reordering arrows you can reorder your answer.

  2. The plus button indicates the new answer that can be filled in.

  3. Click on the exclamation icon button to set the answer as non-conforming, it will highlight the exclamation mark button in orange when turned on:


    Click on the orange button to remove the non-conformity, the button will appear colourless again.

  4. Click on the comment text bubble icon to predefine this answer, it will highlight the text bubble icon in grey:


    Click on the grey button to remove the predefinition of the answer, the button will appear colourless again.

  5. Click on the red trash can button to delete an answer.

  6. Click on the green enter button to add an answer.

Note: It is not possible to enter two identical answers for a question – an error will appear in the top-right corner stating that the "Option already exists".

Setting Non-Conformities

Non-conformities can be set for the following answer types:

  • Free Number

  • Multiple Choice

  • Single Choice – Text

  • Single Choice – Number

Note: For answer types such as Free Text or Date, non-conformity settings are not available.

Example 1: Free Number

When using the "Free Number" answer type, you can define:

  • A min/max input range (e.g. user must enter a number between 23 and 90)

  • A non-conformity range within that (e.g. values between 35 and 48 are flagged as non-conform)

Example 2: Single/Multiple Choice

For "Single Choice questions" or "Multiple Choice questions", you can mark specific answers as non-conformities:

Note: If a person selects or enters a value that falls within the defined non-conformity, this will be indicated on the submitted form.

Auto-Save and Drafts

All progress in the builder is automatically saved.


To save as a draft, simply close the Form Template Builder. You can close the Form Template Builder by clicking on:

  • The button "Back to all templates" button in the top-right corner, or

  • By clicking the "Forms" button in the templates section in the left-panel:

Publishing the Template

Once you're done building the form template, click "Publish Template" to make it available for your colleagues to use:

After clicking on the button, a pop-up will appear displaying the name of the template and offering two options:

  • By clicking the "Replace existing template" button, you will remain within the same template. Your changes will be applied, and the updated version will simply replace the previous one without creating a new template.

  • By clicking the "Save as new template" button, a copy of the template will be created, and the changes that you made are only going to be effective in the new template that was created. The original template you were working on will stay unchanged.
